Ranson's criteria

At presentation,

  "LEGAL"

  • Leucocytes >16000
  • Enzyme AST >250
  • Glucose >200
  • Age >55
  • LDH >350

Within 48 hours,

   "C.HOBBS"

  • Calcium < 8mg/dL  
  • Hematocrit fall of >10%
  • PaO2 < 60 mmHg
  • Base deficit > 4 mmol/l
  • BUN rise > 5 mg/dL
  • (Fluid) Sequestration > 6 litres

 

Ranson's criteria is used to predict the severity of acute pancreatitis. It is classified as severe when 3 or more factors are present.

Hello, I find your article very interesting and very helpfull! Though by browsing for “Ranson’s criteria” I found an article in wikipedia that contains also another point:
“BUN increased by 1.8 or more mmol/L (5 or more mg/dL) after IV fluid hydration” within 48 hours.
I know wikipedia is not completely to trust, so I would like to ask if it is actually a valid point….
You mentioned "BUN rise > 5". So wich one applies? Or do both apply?

Was a 2 marks que for exam...there is more to this though. whats is covered by mnemonic is the criteria at time of admission.

At 48hrs, we check base deficit, S. Ca, Hematocrit, Pao2, Fluid loss and BUN change.
