Rheumatic fever diagnosis: Revised Dukett Jones crirteria

"Arthitis Can Skip Every Where! Legs Around Patella Are Frequently Preferred!"

According to revised Jones criteria, the diagnosis of rheumatic fever can be made when two of the major criteria, or one major criterion plus two minor criteria, are present along with evidence of streptococcal infection. The minor criteria include- arthralgia, fever, heart block, leucocytosis, raised ESR or CRP and history of rheumatic fever.

Goodpasture's Syndrome

'GOODPASTURE'S SYNDROME'

It is an uncommon but aggressive disease with rapid progression

Goodpasture's syndrome is an aggressive disease with a mortality rate as high as 95% earlier. Now it has a better prognosis with modern treatment.
Protein A immunoadsorption is successful in those who do not respond to to plasmapheresis.
Prognosis worst in those with DDRF, oliguria and serum creatinine > 5.7 mg/dl.

IgA protease-producing bacteria

"Nice Strip of Ham"

  • Neisseria (N. gonorrhoeae, N. meningitidis)
  • Streptococcus pneumoniae
  • Haemophilus influenza

Immunoglobulin A (IgA) is the second most common immunoglobulin in human serum (after IgG) and is the predominant immunoglobulin found in mucosal secretions.
IgA proteases act by cleaving the heavy chain of the human IgA1 at specific sites within a 16-amino acid proline-rich polypeptide in the hinge region. This sequence is not present in IgA2 which is, therefore, resistant to IgA proteases.

VDRL: Causes of chronic biological false positive reactions

' SLIM  He-ART '

  • SLE - Systemic Lupus Erythematosus
  • Lepromatous leprosy
  • IMN - Infectious Mononucleosis
  • Malaria
  • Hepatitis
  • Autoimmune diseases
  • Relapsing fever
  • Tropical eosinophilia

In VDRL, serum from the suspect is mixed with the cardiolipin antigen on special slide and rotated for 4 minutes. The cardioloipin antigen remains as uniform crystals if the serum is normal and will form clumps in positive serum (due to reaction with the antibody present in positive sera). As cardiolipin antigen is present both in Treponema pallidum and mammalian tissues, antibodies can be induced by both which accounts for Biological False positive reactions (BFPs) and forms the major disadvantage of this test.
