Apex beat is defined as the lowermost and outermost point of definite cardiac impulse.

It means, you have to locate the lowermost part of cardiac impulse on the chest first and then trace it to the outermost (lateral most) point where you can feel the impulse.

And, it is not the other way around, ie., outermost and lowermost point.
